How to Apply

You may apply in one of two ways:

A) Apply Directly to Northwestern University’s Doctor of Audiology program:

  1. Northwestern Application:  Submit the application form [pdf] and nonrefundable application fee.  The application fee is $75.  Make check or money order payable to “Northwestern University”.  All application materials must be received by February 1.   Return the application to:

  1. Statement of Purpose:  Enclose a one page statement of purpose commenting on any or all of the following:  plans you have to use your education and training; the needs and/or challenges you perceive as important in your field of study; and any personal qualities, characteristics, and skills you believe will enable you to be successful in your chosen field of study.
  2. Essay:  In addition to your statement of purpose, please attach a response to the following question.  Your response should be no more that one page in length.  QUESTION:  What is one aspect of clinical audiology (e.g., a specific etiology, diagnostic technique, rehabilitation approach) that interests you most and why?  Please do not use anecdotes as part of your answer.
  3. GRE Scores:  Have an official report of Graduate Record Examination (GRE) scores sent to Northwestern University (institution code 2550, do not use a department code).  The GRE must have been completed within the past five years
  4. TOEFL: Applicants whose native language is not English must submit Test of English as a Foreign Language (TOEFL) scores (Northwestern University institution code 2550, do not use a department code).
  5. Letters of Recommendation:  Two letters of recommendation are required. Please have your letters of recommendation sent directly to the Doctor of Audiology Program address (above). The required letters of recommendations should be from professors or an academic advisor; additional recommendations are welcomed. Academic referees are requested to compare the student's performance to others in their cohort. Recommendation letters should be printed on official letterhead and contain the original signatures of your recommenders.  Be sure to have your recommenders sign the envelope across the envelope seal.  Submit your unopened letter of recommendations.  Recommendations should be from individuals who can attest to your potential for success in graduate study.
  6. Transcripts:  Have official transcripts sent to the Doctor of Audiology Program address (above) from the registrar of each college or university attended.  Note: A final transcript reflecting completion of an undergraduate degree must be sent before enrolling in the Doctor of Audiology Program.
  7. Optional: Submit a brief resume.

The Doctor of Audiology degree program is administered by the Department of Communication Sciences and Disorders in the School of Communication.  Send the application, fee, and supporting documents to the above address by February 1st.

B) Apply through CSDCAS

The Council of Academic Programs in Communication Sciences and Disorders Centralized Application Service (CSDCAS) is a web-based application service that may be used to apply to the Northwestern University Doctor of Audiology program.  Using CSDCAS may streamline the application process if you plan to apply to several programs.

  • Required Materials: All application materials (items 2 through 7 above) should be sent directly to CSDCAS with the following exceptions. GRE (item 4) and TOEFL (item 5) scores should be sent by ETS directly to Northwestern University, Doctor of Audiology program using institution code 2550.
  • FAQ’s.  CSDCAS posts Frequently Asked Questions on the applicant portal which can be accessed before an application is created.  Please read the FAQ’s before submitting your application.
  • CSDCAS Application Deadline.  All Application Materials must be received by January 15, 2010.
